Chapter 1 - Another Night Shift
The operating room was finally quiet.
After fourteen hours beneath impossibly bright surgical lights, the rhythmic beeping of monitors had faded into the background, replaced by the soft sounds of nurses cleaning instruments and anesthesia beginning to wake the patient.
Dr. Olivia Bennett peeled off her gloves with practiced efficiency.
“Vitals are stable,” the anesthesiologist said.
Olivia nodded once, exhaustion settling into every muscle.
“Good work, everyone.”
The team slowly dispersed.
She pulled off her surgical cap, running a hand through the loose strands of blonde hair that had escaped hours ago. There wasn’t enough coffee in Boston to fix the pounding behind her eyes, but the surgery had gone well.
That mattered.
She stepped into the hallway outside Operating Room Three and leaned briefly against the cool wall.
Fourteen hours.
She couldn’t even remember what time she’d eaten.
Had she eaten?
Her pager answered that question before she could think about it.
TRAUMA ALERT. LEVEL ONE. ETA: 3 MINUTES.
Olivia closed her eyes for exactly one second.
Then pushed herself away from the wall.
“Of course.”
She headed for the elevators, already pulling a clean trauma jacket over her scrubs.
Blackthorne Medical Center never slept.
Neither, apparently, did she.
—
The emergency department was already in motion when the elevator doors opened.
Paramedics rushed through automatic doors.
Nurses prepared medications.
Respiratory therapists checked equipment.
Every person knew exactly where they needed to be.
Controlled chaos.
Olivia loved it.
Most days.
She strode toward Trauma Bay One.
“What’ve we got?”
Emma Sullivan barely looked up from the chart in her hands.
“Thirty-two-year-old male. Motor vehicle collision. Multiple rib fractures, suspected splenic injury, hypotensive on arrival.”
Olivia nodded.
“Where’s—”
She stopped.
Standing beside the stretcher was Dr. Ethan Walker.
Sleeves rolled to his forearms.
Dark hair slightly disheveled.
Stethoscope hanging around his neck.
He looked far too awake for three in the morning.
He glanced over his shoulder and smiled.
“There she is.”
Olivia sighed.
“You couldn’t wait five minutes?”
“I already stabilized him.”
“I noticed.”
Ethan handed a nurse another order before turning back to Olivia.
“Blood pressure’s responding. FAST exam suggests abdominal bleeding.”
She glanced toward the ultrasound monitor.
“You called Surgery?”
“I called you.”
She stepped closer to the patient, quickly assessing him herself.
Pulse.
Pupils.
Airway.
Breathing.
Everything Ethan had said checked out.
Annoyingly.
“You started transfusion.”
“He needed it.”
“You should’ve waited.”
“He would’ve arrested.”
She shot him a look.
“I would’ve preferred making that decision.”
He folded his arms.
“I prefer keeping patients alive.”
The trauma bay fell suspiciously quiet.
Emma looked down at her chart to hide a smile.
Another nurse suddenly became very interested in arranging gauze.
Olivia crossed her arms.
“Stop taking my patients.”
Ethan’s grin widened.
“Stop stealing them.”
A collective sigh rippled through the room.
Emma muttered just loudly enough for everyone to hear,
“They’re starting early tonight.”
Someone snorted.
Olivia ignored them.
Mostly.
She stepped toward the patient again.
“We’re taking him upstairs.”
Ethan nodded.
“I figured.”
She frowned.
“Then why are you still here?”
He looked genuinely offended.
“I wanted to make sure you didn’t yell at anyone else first.”
“I don’t yell.”
Emma raised an eyebrow.
The entire trauma team stared at her.
Olivia looked around.
“What?”
No one answered.
Ethan cleared his throat.
“I’ll let you get to surgery.”
“Please do.”
As he started walking away, he paused beside her.
Quietly enough that only she could hear, he said,
“Nice save upstairs, by the way.”
She blinked.
“You heard about that?”
“The whole hospital heard about it.”
A beat of silence.
“The patient was lucky.”
Olivia looked back toward the trauma bay.
“No.”
She adjusted her gloves.
“They deserved a team that didn’t give up.”
For the first time that night...
Ethan’s smile softened.
Not teasing.
Not smug.
Just... genuine.
“I know.”
The moment lasted barely a heartbeat.
Then another pager sounded.
Another ambulance arrived.
Another emergency demanded their attention.
The softness disappeared beneath professionalism.
Olivia turned toward the waiting stretcher.
“Let’s move.”
Within seconds, the trauma team pushed the patient toward the elevators.
Ethan watched them disappear.
Emma walked up beside him, folding her arms.
“You know she likes arguing with you.”
Ethan laughed.
“I’m pretty sure she doesn’t.”
Emma smiled knowingly.
“No.”
She watched the elevator doors close.
“I’m pretty sure that’s exactly why she does.”
Ethan shook his head, still smiling to himself.
Across the hospital, Olivia was already scrubbing in for another surgery.
Back in the emergency department, another trauma alert echoed overhead.
The night was far from over.
At Blackthorne Medical...
It never was.
