The concept is good, but you have quite a few grammar mistakes, mostly missing commas and "your" instead of you're, so I'd watch out for that. I understand what the poem is saying though, that's a good thing. If you want to improve your writing, you could try to edit it with Grammarly, it's free and very helpful. I like that in this book are more poems, looking forward to read them.
Oh, one more thing. If someone blocks you, then you don't have to apologize, it's them who should. The worst part is that he/she will blame you for their mistakes... and even if they find someone new, you can be glad you're not with them anymore. (Because they will treat the new person the same way they did with you), just remember you didn't do anything wrong.
