Summary

In a palliative care facility just outside Amsterdam, Elena Voss has secured something no one else has dared: a strictly limited one-year trial to integrate sexual intimacy into end-of-life care. Only six patients will be accepted. Every request must pass rigorous medical and psychological screening. Every encounter must be approved by a senior medical board. The workers are carefully chosen, professionally paid, and required to meet each patient with steadiness rather than performance. For some, it is the chance to experience something long denied. For others, it is the opportunity to release decades of shame before time runs out. But as the trial unfolds, a quieter question begins to surface: if we accept that people should retain autonomy over their bodies at the end of life, why do we still treat sexual dignity as the one need we are permitted to ignore? What Desire Remains is not a story about pleasure as spectacle. It is a story about what it means to remain fully human when time is running out, and what it costs the people who carry that responsibility.

Preface

This book is set in the Netherlands.

The choice of location was deliberate. The Netherlands has long been at the forefront of discussions around bodily autonomy, particularly in the context of end-of-life care. It was one of the first countries to legalize euthanasia under strict medical and ethical guidelines, and it maintains a relatively progressive legal framework regarding personal choice and dignity in the final stages of life.

It is important to state clearly from the outset, however, that the central premise of this novel, a regulated institutional programme allowing sexual intimacy as part of palliative care, does not currently exist in Dutch law or medical practice. While there are ongoing discussions in some European countries about sexual rights for disabled people, and while sexual assistance services exist in limited forms in certain contexts, the specific model explored in this book remains fictional.

What Desire Remains is not an attempt to portray current reality. It is a speculative exploration. It asks: if a society has already accepted that individuals should retain meaningful autonomy over their bodies at the end of life, including the right to relief from unbearable suffering and, in some jurisdictions, the right to choose the timing and manner of their death, then what might it look like if that same principle were extended to include sexual dignity and intimate connection?

This is a work of fiction. It does not advocate for policy change, nor does it claim to represent the views of any medical, legal, or governmental body. Instead, it examines the emotional, ethical, and human consequences of such a possibility through the lives of a small number of people facing the end of their lives.

The story that follows is told with care, but without reassurance.
